Orthopedics

What is Orthopedics?

The department of Orthopedic provides treatment about every anatomical structure that forms the arms, legs, and spine.

Basically, arms, legs, spine are consisted with skin, fat and fibrous tissue, bone, muscle and tendon, nerve, blood vessels. And plus, the articular capsule, ligament that surrounds the joints and joints between bones.

You should receive medical treatments at the department of Orthopedics when you experience disorders in the above regions.
There are various major causes of Orthopedic diseases: external injury, infection, metabolic disorders, congenital malformation, tumor, degenerative arthritis and osteoporosis, degenerative disease such as arteriosclerosis, rheumatoid arthritis, inflammatory disease etc. Since the vast range of relevant field, we subdivide the medical service by the disease occurrence parts.

About joint sugery

Knee joint diseases

01 Joint Inflammation

What is joint inflammation?

The joint works as a connecting link between bones, and facilitates the bone movement.
There are various reasons of joint inflammation such as the worn out cartilage because of aging, congenital joint abnormality, strained joints by excessive exercise and obesity etc.
As joint inflammation occurs, you may experience symptoms like stiffness or pressure in the joint, edema. If the pain worsens, symptoms may even occur when resting.

We sort the joint as cartilage, bursa, tendon, mucous cyst, muscle. When one or other of these occur problems, we call this disease joint inflammation.

Symptoms

1) Severe pain caused when walking, sitting, and standing.
2) Joints tend to swell or produce crunching sound.
3) Doesn’t bend well.
4) Show symptoms such as bowed legs.
5) Can’t sleep because of the pain in the knee and thighs.

02 Semilunar cartilage injury

What is semilunar cartilage injury?

A semilunar cartilage is located inside the knee joint, between the thigh bone and leg bone’s joint facet. It has a structure that processes important functions such as conveying weight, dispersion of external force, protecting the articular cartilage, the stability and lubrication of the joint. Semilunar cartilage injury occurs when the knee is twisted in a slightly bent situation.

Although it has high occurrence when excessive exercise, many patients also experience semilunar cartilage injury because of osteoarthritic changes by aging. As for the latter case, many don’t visit the hospital after experiencing a particular injury.

Symptoms

May experience pain and edema at the knee joint. It can even fill with blood in an acute phase. Since there are joint movement limits, you may feel stiffness at certain times when bending and stretching your knees. And also feel instability in the knee, which makes the knee lethargic when going down the stairs or a downhill load.

03 Anterior cruciate ligament rupture

What is anterior cruciate ligament rupture?

As one of the 4 ligaments that forms the knee, the anterior cruciate ligament contributes in joint stability by stopping the shin bone moving forward the thigh bone and limiting the spinning and hyper-extension of the shin bone.
Most of the anterior cruciate ligament rupture patients visit the hospital because of non-contact injuries such as distortion, deceleration injury, hyper-extension movement, but some visit because of contact injuries caused by abductions. Injuries occurred by contact normally has massive associated injuries in the other knee structure. In this case, semilunar cartilage injury is commonly occurred.
If ligament damage is not diagnosed at an early stage, it may cause joint instability and associated injuries in the surrounding soft tissues, osteoarthritic changes in the articular cartilage for a long time. So an accurate diagnosis at an early stage is crucial.

Symptoms

1) Think you’ve heard a whacking sound, and feel lancinating pain in your knee.
2) It feels like you have a problem at your knee.
3) If your experiencing the feeling of as if your knee is falling apart, it may be anterior cruciate ligament rupture.

04 Chondromalacia patella

What is chondromalacia patella?

Patella(kneecap) works as a shock absorber of our body weight. When the external shock is conveyed directly to the knee joint it damages the patella, this affects the knee joint and causes pain. We call this, a chondromalacia patella.
The damaged patella’s surface gets rough and the color turns dark. As the ligament that was covering the joint gets tender, it loses its elasticity and cannot endure the various shocks.

Symptoms

1) Many of the patients are young women.
2) You feel helplessness at the knee joint, and when you move them, it causes overall pain.
3) Pain occurs at the knee joint when you’ve spent a long time sitting with your knees bent, and when you’re coming down the stairs or downhill road.
4) Pain elaborates when crouched down or bending your knees.

Shoulder joint diseases

01 Frozen shoulder

What is frozen shoulder?

A disease that makes the shoulders hard to move, which causes adhesion by an infection at the fibrous capsule called the articular capsule surrounding the shoulder joints.
The shoulder itself has no internal disease, but it makes active or passive motor disturbance and severe pain when the joints move. Since many middle-aged patient’s shoulder pain is usually diagnosed as frozen shoulder, it is one of the most misdiagnosed disease that cannot get proper medical treatments.
Frozen shoulders is a disease that mostly goes through auto-therapy after 12~18 months. In early stage of pain and adhesion, we provide medical treatment about the pain and inflammation. After the pain is relieved exercise therapy that recovers the range of motion by constantly stretching the joints are simultaneously proceeded.
But for radical treatment of secondary frozen shoulders, you should receive medical treatments on diseases like tendon rupture, calcific tendonitis which causes the secondary frozen shoulders.

02 Rotator cuff damage

What is rotator cuff damage?

The 4 tendons that covers the shoulders are called rotator cuff. This helps the smooth functioning of the shoulder movements such as supporting the shoulders or lifting the arm.
Rotator cuff damage is the most common cause of shoulder pain, it occurs when you apply abnormal force at the tendon or experience osteoarthritic changes, as it makes degeneration and weakness in the tendon this leads to the rupture of rotator cuff. When you experience a rupture in the tendons, it is classified with frozen shoulder since it tends to limit the movement of the joints and cause severe pain and weakens the muscular strength.

Symptoms that caused by an early stage of rupture inflammation or edema is treated by non-steroidal anti-inflammatory drug, physical therapy, hormone or cartilage injection, and also limit the patient from doing exercise that could strain the shoulders and recommend to stay in stable condition.
We provide regeneration of the damaged tendons by extracorporeal and laser shockwave therapy, PRP(platelet-rich plasma) and exercise treatment that strengthens the rotator cuff. But if the symptoms are severe and the degree of rupture is massive, we perform trimming or suturing on the tendons through joint endoscopy.

03 Damage on the labrum

What is damage on the labrum?

Labrum is a fibrous cartilage (also referred as cartilage bone) that surrounds the shoulder joints, shaped as a ring. As it is a tissue connected to the ligaments, it supports the shoulders from dislocation when joints move.
A rupture mainly occurs in young patients after placing one’s arm before falling, immoderate movement, weight training, throwing a ball, and also can be accompanied with instability.

Symptoms

1) Usually there are no big symptoms, but when you tilt your arms back or hold your arm up, pain occurs.
2) Although the pain is sudden and severe, if you take a rest, it vanishes in a short time.
3) There is this particular pose that causes pain, so you keep avoiding that one particulat pose.
4) You might dislocate your shoulders because of instability, feel an dislocation or hear particular sounds when you swing your arms.

If the symptoms of the damaged labrum is insubstantial, we could proceed a conservative care that could stabilize the shoulders by posture correction, physical therapy, and shoulder muscle strengthening exercise. However as for severe damage, we need to perform a suture that reconstructs the ruptured labrum into its original location by using the joint endoscopy.

Symptoms that caused by an early stage of rupture inflammation or edema is treated by non-steroidal anti-inflammatory drug, physical therapy, hormone or cartilage injection. We also limit the patient from doing exercise that could strain the shoulders and recommend to stay in stable condition.

04 Calcific tendinitis

What is calcific tendinitis?

It is a disease that causes severe pain due to the calcification around the shoulder’s rotator cuff, which is the tendon of the shoulders.
Mainly occurs in women over 40~50 years old, that has started experiencing osteoarthritic changes at the shoulder tendons. Many patients feel a stabbing pain at the ends of the shoulder, and tend to feel the pain become harsher at night time. This may first start with a minor pain, but quickly turn into an acute pain.
